Arizona residents must have one of the following 12 conditions to qualify for medical cannabis use:

Once a Veriheal physician diagnoses a patient with a qualifying condition and fills out the physician certification form, patients must fill out their application in the ADHS Individual Licensing Portal. Applicants must provide a valid driver’s license or Arizona state ID to prove state residency.
The cost to register and receive a medical license in the state is $150 for the initial card and $150 for renewal every two years. However, the state offers a discount for Supplemental Nutrition Assistance Program patients, only charging $75 for the license. Minor patients must pay $350 for an application, which includes a caregiver, and a caregiver application is an additional $200 for adult patients.
The Arizona Department of Health Services (AZDHS) will review the information and email you a digital Arizona Medical Marijuana Card within 14 days.
To apply for a medical marijuana card in the state of Arizona, you must provide a valid driver’s license or Arizona state ID to prove residency in the state. To apply as a patient without a caregiver, you also must be 18 years-old or older, with a physician’s certification that you are diagnosed with one of the state’s qualifying conditions.
